Please Note:

The Privacy Act of 1974 (5 U.S.C. § 552a) requires that Members of Congress or their staff have written authorization before they can obtain information about an individual's case.
You must submit a signed Privacy Release form in order for us to proceed with addressing your request.

Constituents can contact us for assistance in dealing with Federal agencies.

When submitting your Privacy Release form, please include all pertinent information and claim numbers in your correspondence. This includes:

• Your address, home and daytime phone numbers, and email address;
• Your Social Security number for cases involving the Social Security Administration;
• Your VA claim number for cases involving the Department of Veterans Affairs.;
• Your locator ID number, date of international travel, and social security number for cases involving the Passports;
• Your taxpayer identification number for cases involving the Internal Revenue Service and ,;
• Copies of all related documents and correspondences you may have from the involved agency.
