When submitting your Privacy Release form, please include all pertinent information and claim numbers in your correspondence. This includes:
- Your address, home and daytime phone numbers, and email address;
- Your Social Security number for cases involving the Social Security Administration;
- Your VA claim number for cases involving the Department of Veterans Affairs.;
- Your locator ID number, date of international travel, and social security number for cases involving the Passports;
- Your taxpayer identification number for cases involving the Internal Revenue Service and ,;
- Copies of all related documents and correspondences you may have from the involved agency.
